What to Bring to Your Appointment

  • Health insurance cards
  • Medical records that relate to your condition
  • Test results and copies of x-rays or imaging studies (CT scans, sonograms, etc.)
  • List of current medications, including dosage amounts. Include over-the-counter and herbal medications.
  • Any medical forms that need to be completed by your doctor
  • A referral, if required
  • List of your questions
  • Your eyeglasses and/or contact lenses
  • Someone to drive you home. Some of our patients are given eye drops to dilate the pupils that may temporarily cause blurred vision. If you have had these eye drops at another examination, and they did not cause any blurred vision, you may drive yourself.
